CLIENT ACKNOWLEGMENT & CONSENT

I understand that tattooing involves breaking the skin and that infection, scarring, allergic reactions, or other complications may occur if aftercare instructions are not followed.*

I confirm that I have disclosed all relevant medical information and understand that withholding information may increase my risk of complications.*

I understand that tattoo results may vary due to skin type, placement, healing, and lifestyle, and that no guarantees can be made regarding final appearance.*

I understand that tattoos are permanent and that removal or alteration is costly and may not be fully effective.*

I understand that colour fading, ink migration, and changes over time are normal.*

I consent to the tattoo procedure being performed by my chosen artist at Garage Ink.*

I understand this concent form is valid for 12 months. And i will notify my artist if anything has changed before any tattoo procedure at Garage Ink within that 12 month period. *

NUMBING CREAM & TOPICAL ANESTHETICS CONSENT

☐ I understand that the use of numbing creams, gels, sprays, or topical anesthetics can alter the natural condition and behaviour of the skin. I acknowledge that numbing agents may compromise the skin’s integrity and can result in increased swelling or irritation, prolonged healing, a higher risk of infection, ink rejection, fading, or uneven healing. I understand that numbing products do not guarantee a pain-free tattoo and that sensation may return.*

AFTERCARE RESPONSIBILITY

I accept full responsibility for the care of my tattoo once I leave the studio.*

I understand that improper aftercare may affect healing and final results.*

PHOTOGRAPHY & MEDIA CONSENT

I consent to photos or videos of my tattoo being used for portfolio, social media, website, or promotional purposes.*
